Troop Trip and Activity Notification Form

If you have taken the required Trip and Overnight Planning Training, you are approved to coordinate trips following the council guidelines and Safety Activity Checkpoints. Simply submit this notification form:

  • One Month Prior to Trip

    • Day trip - over 100 miles from your regular meeting location (except when you reserve a Girl Scouts of Western Ohio property or participate in a staff-led event)
    • Overnight trip - 1-4 nights (over 100 miles)
    • High Risk Activity
  • Six Months Prior to Trip

    • Extended Trip - 5+ nights
    • International Trip

    Contact your program and partnerships department when planning begins for extended and international trips.

 Troop leader has verified that all trip drivers meet the requirements of the Volunteer Driver Policy, are registered Girl Scout members and have completed the Volunteer Application Process.
 All drivers must be registered Girl Scout members as a troop leader or troop assistant and complete the Volunteer Application Process.

EMERGENCY CONTACT INFORMATION

An emergency contact person is usually the troop leader or another person designated by the troop leader.

 
 
 

An emergency contact person for overnight/extended trips is someone who is not going on the trip and is designated to contact parents/guardians if there is an incident or change in travel plans.

 
 
 

By signing and submitting this form, I acknowledge that as the troop leader, I am responsible for:

  • Meeting training requirements.
  • Collecting written parent permission, health histories and medication permission.
  • Ensuring all participating adults have completed the Volunteer Application Process and are registered Girl Scout members.
  • Ensuring all girls participating are registered Girl Scout members.
  • Following all safety activity guidelines including acquiring additional insurance and certificates of insurance when necessary.
